To Obtain Quotes for Group Health Insurance...
We need to gather some basic information about your
company. Accurate information is very important in obtaining
accurate quotes.
To request a group medical quote for your company:

- Complete the following forms and fax or email them to our
office:
- Risk
Assessment form - For large and small groups
- Small Group
Medical Census Form - This form is for
small groups of less than 50 employees
- Large Group
Medical Census Form - This form is for large groups of 50+ employees
- The following information, although not required, would be
very
helpful and can be emailed or faxed as well:
- Most recent carrier billing statement (if group coverage
is in place)
- Renewal rates for any existing plans
- Plan summary for each plan in place
Once we receive your information (completed forms above) we
will contact you to acknowledge receipt, clarify any information
provided and request any additional information, if necessary.
We will then obtain competitive bids from carriers and provide
you recommendations based upon the initial criteria you provided.
