census forms) Excel Benefits, Inc. requests individually identifiable information from its users.

code, telephone number, email address, and subject of inquiry (i.e., quote requests). This
information is necessary for the conduct of our business and is shared with business associates
for the purposes of fulfilling the services requested by you. The information is not sold to third
parties, nor is it provided to persons not involved (no direct business relationship to the services
you are requesting) in this process. Personal information is retained by our company for use in
future contact with you for purposes such as account servicing or notification of the availability of
additional value added services which would benefit you.
Personal health information that is provided by you to our company, such as current or past
health conditions, current treatment for health conditions, etc., is provided to business

Log files are simply transaction records that web servers maintain. These log files are used for
